Our Story

Since the Minassians opened the doors of Crestwood Nursing & Rehabilitation in 1978, generations of the family have continued to own and operate the center under the same guiding philosophy: to provide the highest quality care to residents in a home-like setting. Over the years, we have continued to transform our Warren, Rhode Island, location to accommodate the evolving clinical, social, physical and spiritual needs of residents. Today, we offer 76 private and semi-private rooms, skilled nursing services, a range of therapies, hospice, dementia and Alzheimer's care, a 220-plus square foot rehabilitation center and more. But one thing hasn’t changed – our commitment, compassion and care.

What separates us from the rest

Crestwood delivers the highest clinical quality and excellence, and is continually recognized for the top care it provides residents.

  •  A Federal and State Licensure survey conducted in 2016 found Crestwood deficiency-free. The survey, performed by the Center for Health Facilities and Regulations, determined Crestwood was in compliance with Federal participation requirements for nursing facilities participating in the Medicare and/or Medicaid programs.

  • US News & World Report awarded Crestwood Nursing & Rehabilitation five stars – the top rating – for Quality Measures. The recognition is based on the percentages of residents who got recommended care such as flu vaccinations, and percentages of residents who had pain, bedsores, urinary tract infections, and other care-related problems.

  • Medicare has assigned Crestwood five out of five stars in its Quality Measures Rating. This rating takes into account different measures of the quality of care that residents receive. Examples of data considered include the prevalence of pressure sores among residents, as well as improvement in residents’ ability to move around.

  • Rhode Island Department of Health, in its Nursing Home Summary Report last updated in May 2016, gave Crestwood Nursing & Rehabilitation five stars – the highest rating – in Quality Measures.
